Objectives

• Design and implement post-hospitalization follow-up care plans for patients with psychiatric conditions to ensure continuity and safety.
• Identify and mitigate stigma and barriers affecting mental health assessment and access to care.
• Conduct a structured mental status examination to support accurate psychiatric evaluation.
• Support the ongoing mental health stability, treatment adherence, and quality of life in a culturally competent manner.
• Apply culturally competent care practices while respecting patient’s wishes regarding end-of-life care.
